Repair Form

If your phone is dead or you are experiencing problems with your phone, use another campus phone and dial 4357 (HELP) and leave a message.

In your message, include the following information:

  • Name
  • Department
  • Contact Extension
  • Problem Extension
  • Phone type (18 button, 30 button, or Voice Over Internet Protocal)
  • Location of problem
  • Do you have dial tone?
  • Description of problem

 

Repairs received before 2:00 p.m. will be handled the same business day. Repairs received after 2:00 p.m. will be handled by the next business day.
