very important to show it to your doctor or hospital during each visit. |

Read the CSU Health Link Insurance Brochure
(available in our office and online at www.csuhealthlink.com) and become familiar with
your health plan. Schedule of Benefits will give you
a summary of the benefit coverage. |

ALWAYS go to a Preferred Provider for medical care.
Anthem Blue Cross of California is a very large health organization comprised of thousands of doctors who have
chosen to become part of their preferred provider network.
- There is a $100 annual deductible. The copay for doctor visits are $10
per visit, and the emergency room copay is $50 when you go
to a Preferred Provider.
- Charges will be covered at 100% of the preferred allowance when you go to a Preferred Provider.

You will be reimbursed 50% of actual costs for prescription medicines by the
Insurance Claims Administrator (BC Life). Claim forms are available in our office and online at
www.bluecrossca.com. Keep copies of all claims you
submit for your records. |

For basic illness/injury and routine physicals, students are encouraged to use
the CSUF Student Health and Counseling Center (SHCC) on
campus. Many of their health services are free of charge and counseling services are also available.
