Required Information for Contact Reports

Please include the following information for contact report data entry. Please note that a contact report will not be filed until all this information has been received.

Date:

  • date that contact was made, not date of entry

Contact Type:

  • 3rd Party Info
  • Correspondence
  • E-mail
  • Event
  • Group Meeting
  • Phone Call
  • Visit on campus
  • Visit off Campus

Purpose:

  • Introduction/Inquiry
  • Assignment
  • Cultivation
  • Pre-Proposal
  • Solicitation
  • Stewardship
  • Volunteer
  • Sponsorship/Marketing
  • Planned Gift Dialogue

Description:

  • Descriptive title for contact report that is no more than 115 characters. The description will be reported out in the weekly contact report distribution list.

Text:

  • Any information that is relevant to the discussion and or visit. The text section should also indicate if there are attachments and or action items (examples: follow up phone call, address, phone or email update).

Attachments:

  • Attach all documents (word, excel, PDF) that should accompany the contact report.
