Required Information for Contact Reports
Please include the following information for contact report data entry. Please note that a contact report will not be filed until all this information has been received.
- date that contact was made, not date of entry
- 3rd Party Info
- Group Meeting
- Phone Call
- Visit on campus
- Visit off Campus
- Planned Gift Dialogue
- Descriptive title for contact report that is no more than 115 characters. The description will be reported out in the weekly contact report distribution list.
- Any information that is relevant to the discussion and or visit. The text section should also indicate if there are attachments and or action items (examples: follow up phone call, address, phone or email update).
- Attach all documents (word, excel, PDF) that should accompany the contact report.
