One Semester Extension Form Click here for the One Semester Extension Form: Participants may request to extend the semester policy one additional semester. Those who wish to do so must either use the on-line version of the application, print the PDF file version of the application, or pick up a copy of the application in the Dean of Students Office, Titan Student Union Room 235. Experiential Component Intent Form Click here for the Experiential Component Intent Form: Participants are required to complete a minimum of 5 hours of applied leadership. This can be pretty much any activity that participants are involved outside the classroom or workshop setting. Five hours are required for each track the participant is involved in. Participants must submit this form for approval by the SLI Coordinating Committee before April 11, 2008 in order to receive a certificate at the 2008 SLI Recognition Banquet. Experiential Component Verification Form Click here for the Experiential Component Verification Form: Participants are required submit this form with their 1-2 page reflection paper (see Intent Form for policy) after the completion of their applied leadership experience. This must be turned in by April 18th at 5:00pm in order to receive a certificate at the end of the year recognition banquet.
