Applying for Disabled Student Services
How do I apply if I have a Learning Disability (LD) or Attention Deficit/Hyperactivity Disorder (AD/HD)?
The application file for a student with a learning disability consists of three documents: an Application for Services, a Confidential Screening Questionnaire, and a copy of the most recent testing results for the Learning Disability. The application and questionnaire can be found under the FORMS section of this website. The testing results must have been provided by a certified professional within the past five years. After you submit the three documents to our office, it will take two to three weeks for our office to contact you for an intake appointment with one of our LD/Mental Health Specialists.
Students who have testing results older than five years, or who have never been tested for a learning disability or AD/HD, may be subject to testing. We may offer testing services to enrolled students. However, due to the high demand for learning disability testing, we are usually able to test a few students each semester. To seek testing outside of the University, you may consider contacting Argosy University at 714-338-6248. They currently charge $100 for testing services.
How do I apply if I have a Psychological Disability?
The application file for a student with a psychological disability consists of three documents: an Application for Services, a Confidential Screening Questionnaire, and a Disability Verification Form completed by a licensed professional. These documents can be found under the FORMS section of this website. The Disability Verification Form can be substituted with a physician’s note, testing summary or medical record indicating the disability diagnosis. After you submit the three documents to our office, it will take two to three weeks for our office to contact you for an intake appointment with one of our LD/Mental Health Specialist.
How do I apply for disabled services if I have a Physical, Other Functional, or Temporary Disability?
The application file for a student with a physical, other functional, or temporary disability consists of two documents: an Application for Services and a Disability Verification Form completed by a licensed health professional. These documents can be found under the FORMS section of this website. The Disability Verification Form can be substituted with a physician’s note, testing summary or medical record indicating the disability diagnosis. After you submit the two documents to our office, it will take one to two weeks for our office to contact you for an intake appointment with one of our counselors.
