	Address Update Form Please click here for the online form for updating your address information. All F1 and J1 international students are required by U.S. law to notify their school of any change in U.S. address or home country address.

	Alternate Health Insurance Petition Form — Refer to the frequently asked questions for information on alternate health insurance. Deadline for Fall Semester: June 1 Deadline for Spring Semester: October 1

	Bank Certification — This is an optional form to be used along with required International Student Financial Affidavit form.

	Concurrent Enrollment Form — (.pdf) Refer to the concurrent enrollment guidelines found on the Academics section of this website for more information. Use the Reduced Course Load Form and check item B-Concurrent Enrollment. You must submit enrollment receipts of the other college together with your Reduced Course Load Form. Deadline: Fall 2008 - September 2, 2008

	Financial Disclosure Form — This form is required when applying for International Student scholarship. Click here for more information.

	Health Insurance Refund Request Form — Please read the form carefully before submitting it. Deadlines for submitting refund request are as follows: Fall semester: August 31 Spring semester: February 28

	International Student Financial Affidavit Form — The completion of this form is required in order for the IEE office to process a Form I-20 (F-1) or DS2019 (J-1).

	International Student Scholarship Application — This form is required when applying for International Student scholarship. Click here for more information.

	Non-Enrollment Notification Form Follow the link to fill out this form online.

	On-Campus Employment Verification and Statement of Agreement — In addition to the application form available at the Social Security Administration Office, this form is required when applying for a social security number, in order to accept on-campus employment. Please refer to the guidelines for "Social Security Card" for additional documents needed.

	Program Extension Request — (.pdf) Please check with the IEE Office for the deadline for submitting this form. When submitting this form, you must include an updated International Student Financial Statement & Affidavit form with supporting financial documents dated within the last six months.

	Reduced Course Load Form — (.pdf) Refer to the reduced course load guidelines for more information. Submission deadline for Fall 2008 is September 2, 2008

	Transfer Out to Another School Request — After you have been admitted to another school, complete this form and bring it with your acceptance letter from your new school to your meeting with the CSUF International Student Advisor. Refer to the transfer out guidelines for more information.

	Volunteer International Peer Program Application — (.pdf) Complete this form if you are interested in volunteering with the International Education & Exchange Office.

	World Citizen Award Application — Award presented by International Friendship Council to recognize a graduating student from Cal State Fullerton.
