2. You may be required to submit a Leave Of Absence Request Form to the IEE Office

I am graduating at the end of this semester. I will leave the US and I will not apply for post-completion OPT.
1. Please submit a copy of plane ticket to IEE Office

I am currently on approved post-completion Optional Practical Training (OPT) but have decided to return to my home country before my OPT ends
1. You must contact the IEE office if you are planning to return to your home country
2. Please submit a copy of plane ticket and a copy of OPT card

I am transferring to attend another U.S. school
1. Please submit a copy of your Admission letter from your new school and a Transfer Out Request Form

I am returning to my home country and will not continue my academic program at CSUF
