REGISTERING WITH DISABLED STUDENT SERVICES
• New students who have not had
accommodations at CSUF or at a previous college complete
(blue) Application for Services ( Attachment #1).
New students who have a learning or visual disability
will also complete the Confidential Screening
Questionnaire (Attachment #2) for in-take purposes.
• Students who have had accommodations at previous
institutions complete (yellow) Disability
Verification Form (Attachment #3) for release of
information purposes.
• Students requiring services will also need to complete
Student Support Services Form (Attachment #4)
indicating coursework information for each
semester.
Completed forms are then faxed up to Disabled Student
Services at the main campus: (714) 278-2408.
• Once received, DSS office will review and contact
student to set up an appointment to meet with a DSS
counselor. (The turn around time is usually about a
week).
• Once evaluated and declared eligible for
accommodations, DSS will FAX Letter(s) of
Accommodations to Student Affairs for each of the
student’s professors. Student Affairs retains a file
copy and sends student’s copies to them.
• If they wish, the student may then deliver letter(s)
to each professor.
• During the course of the semester the student may wish
to use the alternate location for testing. In this case,
they will pick up a TESTING INFORMATION form
(Attachment #5) from Student Affairs which they will
complete Section I which includes Name,
Professor, Course and Day and time test is to be
administered. Section II is completed by DSS
Staff or coordinator from Student Affairs. This section
indicates to the professor the prescribed accommodations
the student receives for exams. The professor then
completes Section III by the instructor and form
is returned to Student Affairs. Students should take
exams at the same time as the rest of the class unless
there is prior approval of professor.
The professor and student need to discuss the time and
date that the student will take the exam. It is
important that the student note all materials allowed to
the entire class. Any special instructions should be
included. (If the testing form is to be used for weekly
quizzes, please indicate such in the Special
Instructions section and the form will be followed for
the entire semester.) It is critical that the professor
complete the sections marked Test Procurement and
Test Return and Delivery and that the form is
signed. This verifies that the professor has reviewed
the entire form and has had a discussion with the
student regarding the exam. If the form is not signed
by the professor, the DSS staff will not be permitted to
administer the exam.
When Testing Information Form has been completed,
it should be returned to Student Affairs to post date of
exam on master calendar and file. It is the
responsibility of the professor to see that exam is
delivered to be administered.
When test is administered, start and ending time are
noted on testing information form. Completed form is
kept on file for at least one semester.
