Medical Records Mission Statement


The mission of the Medical Records Department is to maintain complete and accurate medical records, to safeguard all information against loss and use by unauthorized persons, to maintain medical records in a manner permitting prompt retrieval, and to assist Health Center staff and patients as necessary.


To request medical records for yourself or another provider/health facility:

  1. 1. Print, complete, and sign a copy of the Authorization For Release of Information form.
  2. 2. Submit by mail, fax, or walk in to the Cashier window.

* Processing your request may take up to 14 business days. It may be longer if the chart is archived in our offsite storage area. There is a minimum charge of $2.50 for the first five pages and fifty cents per page thereafter.
