Applying for Disabled Student Services

How do I apply if I have a Learning Disability (LD) or Attention Deficit/Hyperactivity Disorder (AD/HD)?
The application file for a student with an LD or AD/HD consists of the following documents: an Application for Services, a Confidential Screening Questionnaire, and either a copy of the most recent assessment results for an LD or a Disability Verification Form (DVF) signed by a licensed professional for AD/HD. The application, questionnaire, and DVF can be found under the FORMS section of this website. The LD assessment results must have been provided by a certified professional within the past three to five years. After you submit these documents to our office, it will takes approximately three weeks for our office to contact you for an intake appointment with one of our LD/Mental Health Specialists.
Students who have LD assessment results older than three to five years, or who have never been assessed for an LD or AD/HD, may be subject to an assessment. We may offer assessment services to enrolled students. However, due to the high demand for LD and AD/HD assessments, we are only able to assess a few students each semester. To seek assessment outside of the University, you may consider contacting Argosy University at 714-338-6248. They currently charge $100 for assessment services.
How do I apply if I have a Psychological/Psychiatric Disability?
The application file for a student with a psychological or psychiatric diagnosis consists of the following documents: an Application for Services, a Disability Verification Form completed by a licensed professional, and, in some cases, a Confidential Screening Questionnaire. These documents can be found under the FORMS section of this website. After you submit these documents to our office, it will take approximately three weeks for our office to contact you for an intake appointment with one of our LD/Mental Health Specialists.
How do I apply if I have a Physical, Other Functional, or Temporary Disability?
The application file for a student with a physical, other functional, or temporary disability consists of two documents: an Application for Services and a Disability Verification Form (DVF) completed by a licensed health professional. These documents can be found under the FORMS section of this website. The DVF can be substituted with a physician’s letter indicating the disability diagnosis and functional limitations. After you submit these documents to our office, it will take approximately three weeks for our office to contact you for an intake appointment with one of our counselors.
