Residential Conduct and Care Reporting

This page includes two reporting options. The HRE Reporting Form is for housing-related incidents and potential violations of student conduct or HRE lease agreements. The OCRE Reporting Form is for reporting discrimination, harassment, sexual misconduct, and related CSU policy concerns. Both forms are reviewed by the appropriate office, and support is available throughout the process.

HRE Reporting Form

When HRE receives a incident report, an investigation into the case will begin, following  Executive Order 1098 . Involved students and witnesses may be contacted to schedule a meeting. The meeting is an opportunity for the student to explain what happened from the student’s perspective. The meeting is educational, not adversarial, and students are expected to be honest and forthcoming during the process.  

After the investigation is completed, one of two outcomes may occur:

  1. It will be determined that the student did not violate the Student Conduct Code and or HRE leasing agreement and the case will be dismissed.
  2. It will be determined that the student has violated the Student Conduct Code and or HRE leasing agreement and appropriate consequences or sanctions will be assessed.  In cases where it is determined that the student has violated the Student Conduct Code and or HRE leasing agreement the process will proceed to a formal hearing.

OCRE Reporting Form

Students and employees have several reporting options if they may have experienced Sexual Harassment, Sexual Misconduct, Sexual Exploitation, Dating Violence, Domestic Violence, Stalking, Retaliation, or Discrimination or Harassment based on Age, Disability, Gender, Gender Identity, Gender Expression, Genetic Information, Marital Status, Medical Condition, Nationality, Pregnancy or related conditions, Race or Ethnicity (including color, caste, or ancestry), Religion or Religious Creed, Sex (including Sex Stereotyping or Sex Characteristics), Sexual Orientation, and Veteran or Military Status.

CSUF's Office for Civil Rights and Equity is here to help. Any person considering reporting an alleged violation of the  CSU Nondiscrimination Policy  should discuss any concerns about their safety or the safety of others with the Title IX Coordinator/DHR Administrator.

Responsible Employees have a duty to report to the Office for Civil Rights and Equity when they know or have reason to know of allegations or acts that may violate the CSU Nondiscrimination Policy , including Discrimination and Harassment based on a Protected Status, Sexual Harassment, Sexual Misconduct, Sexual Exploitation, Dating or Domestic Violence, Stalking, or related Retaliation.
