Electronic Waste (E-Waste) Disposal Guide

Never dispose of equipment with an asset tag, battery, electrical components, or other components into a dumpster.  Most electronic waste contains materials that need to be disposed of properly and should not end up in the landfill.

Download the form(s)

Start by filling out the Property Survey Form. Each item must be listed on the form separately. 

If the item contains data then download the Data Destruction Form as well.

Request for Property Survey Form

Complete the top half of the form to the best of your ability. Make sure that you or a local tech have properly wiped, reset, and unlocked the items (removing all personal data). If you do not have a local tech or are unable to do this yourself, please fill out the E-Data Sanitation Verification Form for those items.

Important Fields

Fill out as many fields as you can to make the form complete.  Below are fields which require special information which might be missing or hard to find.

  • Property Asset Tag Number
    • Property should be fitted with either a white or silver tag.  Usually it is on the back of the item.  Each tag will have a bar code along with a number that needs to be filled out in the form.
    • If a property tag is missing, add the equipment serial number to the description.
  • Description of Equipment
    • If there is no asset tag please provide the equipment serial number.  
    • Descriptions should be something like:
      "Dell Optiplex 7060 - A5ARW". 
    • Be specific about the equipment and don't just put "Computer" or "Monitor".

Data on the device

If you have user data still on the device it needs to be sanitized.  This includes local accounts and passwords which may be left on the device from yourself or other users.

For the question: Has the equipment been Sanitized?

  • Check Yes: There is no user data on the equipment
  • Check No: There is still user data and needs to be removed.  This is where you need to fill out the E-Data Sanitation Form for devices.

For the question: Has the equipment been Unlocked?

  • Check Yes: All passwords and accounts have been deleted from the device
  • Check No: Not able to delete accounts or unlock the device

Complete the form

Get your department property coordinate to sign and date the form before you submit it for processing.

E-data Sanitation Verification Form

This form is only required for assets that contain data.

All assets must be wiped of data before you send them to us. If possible, have a local tech wipe and remove the drives.

 

If you are unable to do  this, we will remove the drives and send them to IT-Rollout to be wiped and destroyed. Please check the appropriate box for destruction instructions.

Sanitation / Desctruction Method

Choose one of the methods for the equipment to have its data cleaned.  

  • Remove internal media for destruction
  • Data wipe and return media
  • Destroy media
  • Completely wipe and destroy device
  • Other - add instructions in additional comments

Complete the form

Make sure you have fille out the form and signed it.  IT Asset managment will need to approve the destruction of data before the equipment can be cleaned.

Final Steps

After completing the forms with required signatures, email them to datacenter-req@fullerton.edu and specify the day and time you would like to drop them off.

The drop-off location is in the library basement at room PLS-082 on Tuesdays and Fridays during regular business hours.

 

For large quantities of 10 or more items, email datacenter-req@fullerton.edu and we will coordinate a pickup date and time.
