Accessible Furniture Request Form: If you need to request accessible furniture, please submit this completed form to dleischner@fullerton.edu or to the DSS Front Office in UH-101 or email to dsservices@fullerton.edu.

Accessible Short Document Request Form: Complete the form to request accessible format of a short document (syllabus, handout, PowerPoints, class notes, etc.).

Accessible Textbook Request Form: Complete and submit the online form to request accessible format of a textbook. Upon submission, you will receive an email of your Accessible Format Request. Any questions should be sent to ampinedo@fullerton.edu.

Disability Verification Form: If you are applying for services, please upload and attach this completed form to your online application. If you are updating your documentation, please submit this completed form to the DSS Front Office in UH-101 or email to dsservices@fullerton.edu.

Release of Information Form: If you would like your documentation released to yourself or sent to another location, fill out the Release of Information Form and submit to the DSS Front Office in UH-101 or email to dsservices@fullerton.edu.
