Please read - Some of these Microsoft Word documents contain macros. When you select a form from below you may be prompted whether or not to enable the macros attached to the document. The attached macro must be enabled as it is important to the form's functionality. If you do not see a message box prompting you to enable macros and the form doesn't seem to be working properly, close the document then from the toolbar at the top of the MSWord screen select Tools/Macros/Security and toggle the "medium" button.
