Faculty Separations

This page presents information and forms for Full-time faculty only. Human Resources provides a very helpful overview of what is required on the part of the employee and department in order to successfully complete the separation process. Please visit this page prior to filling out the required separation forms by clicking on the link below.

Employee Separation & Property Clearance Information (HR page)

Please note that BOTH the web-based Faculty/Staff Separation On-Line Form AND the Informed Filler Notice of Separation Form must be completed. The Property Clearance Form must also be filled out by the appropriate administrator in the department.
