New ItoA application and Re-Applicant Form Posted

The HPAO is proud to announce that the new 2014-2015 ItoA Application and Re-Applicant Form are now available. The applications can now be downloaded using the "Documents for File" tab located on your left hand side.

Reference Request Guide

Please note that the Reference Request Guide (RRG)  will no longer be required for the 2014-2015 ItoA cycle. Although the form is no longer needed, students are still encouraged to provide their letter writers with the guide as it provides helpful guidance in composing a letter of evaluation. The guide also details the various ways a letter writer can submit their letter to the HPAO.

ItoA TITANium Communities Page

If at any time you would like to be added to our Health Professions TITANium Communities page simply just email with this request and you will be added in about 2-3 business days. Our communities page not only allows students to submit all ItoA materials but also serves as a tracking mechanism to find out what has been received for your file.

Our office is pleased to announce the ItoA Youtube Channel.

11/22/12 Intent to Apply application Please note that changes have been made to the intent to apply application to reflect the most up to date information. To download the form please click on "Document for File" located on the left hand side.
11/19/12 Re-Applicant Form Students who have previously submitted a complete ItoA application in the past are required to fill a re-applicant form found here.

The new application is available to complete! We've generated a PowerPoint presentation to help guide you through the new Intent to Apply application.

A deadline of Monday,  January 13, 2014, for the submission of the required contents of your file is not flexible and will result in the loss of a committee letter. In almost all cases, a committee letter is something to strive for, as it is a reflection of your capabilities and character, and will greatly strengthen your candidacy, in most cases. Please note the loss of a letter can be detrimental to your professional school application process. For additional details about the required documents for your file, please refer to the PowerPoint presentation or the  Committee Letter flowchart.

You also have the option of having the Health Professions office upload a letter packet to the professional schools you're applying. If you'd like to have only a letter packet and not a committee packet submitted on your behalf, please refer to the Letter Packet flowchart. Please note that there is no deadline associated with a letter packet and materials can be submitted at any time.

Once your file has been established, you will be added to a TITANium community were you will have access to track the progress of your file through the grade center. Please be sure to provide each of your letter writers with the Reference Request Guide to assist with composing a letter on your behalf. The guide details the various ways your letter writer can submit their letter to our office.
