Opening a File

• The follow steps are needed to complete your file:
• 1. Open a File Form
• 2. Admission test results
• 3. Copy of your Application
• 4. Letters of Recommendation
• 5. Transcripts
• 6. Autobiography
• 7. Personal Statement
• 8. List of Schools
• 9. GPA calculation sheet
• 10. Signed Waiver
• 11. Photo Waiver
• 12. A 2x2 photograph
• 13. Fee paid

• Open a File form
• Guidelines for Letters of Recommendation
• List of Schools Worksheet
• Waiver of Access to Confidential Information
• Re-opening a File Form (use this file only if you have filled out a file form in the past)

To open a file, e-mail hpadvising@fullerton.edu and request that you be added to blackboard. All of the forms should be uploaded to blackboard. Students can see their progress on the grades page and see what else needs to be turned in to complete their file.
