Forms & Templates
To ensure that complete and accurate proposals to external funding agencies are submitted on time, CSUF faculty and other researchers are requested to contact their OGC Coordinator as soon as a decision is made to submit a proposal. This will ensure that the OGC and pertinent offices can identify all administrative and compliance requirements early in the process. Please see the Grant Submission Timeline Policy Memo for important internal guidelines.
Each Fiscal Year (FY) a version identifying that FY will be disseminated electronically and online to ensure that forms contain up-to-date information and the Principal Investigator, Researcher or Administrator uses the most current version of the campus forms. For example, the Internal Routing Form to be used for the period July 1, 2015 through June 30, 2016 will contain the footer, “FY2015-16,” whether or not there have been any changes in the form.

Internal Routing Form (required for all proposals)
Short Routing Form Template ( Use this form for internal approvals of Material Transfer Agreements, Non-Disclosure Agreements, etc., that do not include transfer of funds)
Budget Template (Please contact your OGC Coordinator for assistance in budget development)
Financial Status Questionnaire Form (for subcontracts that do not have annual A-133 audit)
