New Student Employee Checklist – This form is a checklist to assist the employer in hiring a new student employee.

Student Employee Performance Evaluation - Use this form to conduct your Student Employee Performance Evaluation. The student should be given the original signed evaluation and a copy should be kept in the department.

Sample Forms Students Will Complete at Payroll Services

Confidentiality Form – This form is an agreement between the University and the student that the student will keep all information he/she has access to confidential.

CSU Form SSA-1945 – This form advises the student employees that the earnings from their student employment positions are not covered under Social Security.

I-9 Employment Eligibility Verification Form - This form verifies employment eligibility. The student will need to also bring documents that establish identity and eligibility to work in the United States. Examples of acceptable documentation are found on the I-9 form.

Nondisclosure of Address – A form requesting that a student employee’s home address or telephone number not be public record.

Student Payroll Action Request Form (SPAR) – This form is a multi-purpose document used to make changes to Federal and State Withholding, address changes or name changes, and correct birth dates or ethnic codes.
