COVID-19 Vaccine and Booster Requirements

California State University (CSU)   announced on July 27Opens in new window   that all CSU faculty, staff, and students who are accessing any university location MUST be immunized against the virus that causes COVID-19.

The CSU system also announced on December 22 Opens in new window that all CSU faculty, staff, and students must receive a vaccine booster shot in order to be fully immunized against the virus that causes COVID-19. Booster upload information is forthcoming. 

All students will need to provide proof of COVID-19 vaccination as a condition for participating in on-campus instruction, co-curricular activities, and/or accessing campus facilities.

Proof of vaccination includes a picture or scanned copy of the COVID-19 vaccination card, or equivalent medical record with the following information: name, date of birth, type of vaccine, lot number of vaccine, date(s) of injection, healthcare professional or clinic site.

Check your status and upload your proof of vaccination on the Titan Health Portal.
