この研究は何に関するものですか?

本研究の目的は、日本に住む多様な人々の身体的健康、精神的健康、および健康に関連した行動に最も影響を与える人口統計的要因および社会的要因を明らかにすることです。  

以下の二つの条件を満たしている方に対して、調査へのご協力をお願いしています。 

1. 18歳以上である方、かつ
2. 日本に在住の方。 

日本人でも外国人でも構いません。本国籍の有無、人種、エスニシティ、永住権の有無も問いません。  

年齢の上限はございません。ですので、18〜19歳、20代、30代、40代、50代、60代、70代、80代、90代、100歳以上等の年齢に関わらず、ぜひご協力ください。  

18歳未満の方、または観光で一時的に日本を訪れており日本に居住していない方は本調査にご協力いただけません。  

研究はオンライン上で実施されます。調査への協力には約30分かかります。本調査には、現在ご回答いただけます。調査は協力者が十分な数に達するまで継続いたします。 

本調査に協力した場合、どのようなことが求められますか?

本調査にご協力いただける場合、オンライン調査に回答することが求められます。調査は具体的に、以下のような手順で実施されます。 

• まず匿名のオンライン調査へのアクセスリンクが提供されます 。このリンクは、いくつかの方法で提供されます。メール、電話メッセージアプリ（LINE、WeChat、WhatsApp、Facebook Messengerなど）、またはDiStefano博士あるいはあなたの知り合いからのSMSテキストメッセージで提供されます。他にも、ソーシャルメディア（LINE、X/Twitter、Instagram、Facebook、TikTok など）の投稿や、 Web サイトにリンクが表示される場合があります。 
• 調査へのリンクをクリックすると、最初に表示されるのは今読んでいるものと同じ情報を提供する同意書です。調査への協力に同意する場合は、その画面の下部にあるチェックボックスのあてはまるものにチェックを入れ、2番目の画面に進んでください。  
• 2番目の画面から調査アンケートが始まります。調査アンケートでは、あなたの個人的な特性、身体的および精神的健康、現在および過去の行動と経験、あなたの人生およびあなたの周りのコミュニティに属している人々全般に対する現在のあなたの考えについて質問します。  
• 尋ねられた質問については回答を拒否することもできます。ただし、電子ギフトカードを受け取るチャンスの抽選に参加を希望する場合、アンケートのすべての質問に回答する必要があります。 
• 最後の質問への回答を送信すると、調査協力が完了します。 

あなたのデータは、他のすべての回答者のデータとともに集計されます。集計されたデータは、分析のために統計ソフトウェアプログラムに入力、処理されます。その結果は報告書にまとめられ、公表されることもあります。 
結果の報告を希望する場合は、研究者にメールでその要望を伝えることができます。その場合、研究が完了し、すべての最終結果が利用可能になった段階で結果についての報告を送信いたします。報告をご希望の方は、以下のディステファノ博士の連絡先情報を参照してください。 

本調査に協力した場合、私に何かメリットはありますか? 

本調査に協力することで、あなたに直接的なメリットはありません。しかし、本研究の結果は、日本に存在する多様なコミュニティの社会的および健康上のニーズに対してより良く応えるために利用可能な情報を提供するでしょう。 その意味で、調査に協力することで、他の人を助けることができるかもしれません。 

本調査に協力した場合、私に何かリスクがありますか? 

アンケートの質問の中には不快に感じるものがあるかもしれません。その場合、回答したくない質問には回答しないこともできますし、いつでも調査協力を取り消すことができます。万が一、著しい悩みを抱えるようになった場合は、厚生労働省のメンタルヘルス相談窓口0570-064-556 (フリーダイヤル) に電話することができます。自殺願望が生じた場合、日本語話者は #InochiSOS ホットライン(0120-061-338 )まで、英語話者はTell Lifeline Japan (03-5774-0992) までご連絡ください。他にも、一般的な医学的およびメンタルヘルスの問題、自殺予防、および依存症に役立つ連絡先情報と他のサービスや組織へのリンクについても、オンライン調査の最後の画面で提供します。

調査は匿名で行われます。そのため、あなたが違法行為(薬物使用、未成年者の喫煙、飲酒、ギャンブルなど)やその他の機密情報を調査で回答したとしても、それがあなたや他の誰かに法的、経済的、社会的、あるいはあなたや他の誰かの雇用、または評判、に悪影響を与えるリスクは非常に低いと考えられます。  

私の情報の匿名性または機密性は保たれますか?

本調査に協力するための費用はかかりません。また、本調査に協力することで金銭やその他の報酬を受け取るという保証はありません。ただし、アンケートのすべての質問に回答する場合、ご協力の見返りとして電子ギフトカードを受け取るチャンスの抽選に参加するかどうかを尋ねられます。あなたが無作為に（ランダムに）選ばれた場合、ディステファノ博士の研究チームから電子ギフトカードが記載されたメールが届きます。2025 年には少なくとも 2 回の抽選が行われ、そのたびにギフトカードを受け取る調査協力者が複数選ばれます。

Study Information

What is this study about?

The purpose of this study is to determine which demographic and social factors most affect the physical health, mental health, and health-related behaviors of a diverse sample of people living in Japan.

You are being asked to take part because you have the following two qualifications:

1. You are at least 18 years old; AND
2. You are currently living in Japan.

You may be Japanese or non-Japanese. You may have any nationality, race, and ethnicity; and you may participate regardless of whether or not you have permanent residency status.

There is no upper age limit, so people aged 18-19 and in their 20s, 30s, 40s, 50s, 60s, 70s, 80s, 90s, and 100s are welcome to participate.

You cannot take part in this study if you are under 18 years of age or if you are visiting Japan temporarily for tourism/sightseeing and therefore are not living in Japan.

Participating in the study will take about 30 minutes. The online survey is open now and will remain open until we have a sufficient number of participants.

What will I be asked to do if I am in this study?

If you take part in this study, you will be asked to complete an online survey. Specifically, the following procedures will occur:

• You will have access to an electronic link to an anonymous online survey. You will have access to this link in one of several ways. You might receive it in an email, a message in a phone messaging app (for example, LINE, WeChat, WhatsApp, Facebook Messenger), or in an SMS text message from Dr. DiStefano or someone else you know; or you might see the link in a posting on social media (for example, LINE, X/Twitter, Instagram, Facebook, TikTok) or on an organization’s website that you visit.
• When you click on the link to the survey, the first thing you will see is the same information you are now reading. To provide your consent to participate, you simply will need to tick a checkbox at the bottom of the screen and proceed to the second screen of the survey.
• Starting with the second screen of the survey, you will be asked questions about your personal characteristics, physical and mental health, current and past behaviors and experiences, and current thoughts about your life and the people around you in your community in general.
• You may refuse to answer any question that is asked in the survey. However, if you wish to enter an opportunity drawing for a chance to receive an electronic gift card for your participation, you must answer all the questions in the survey. 
• After you submit your answer to the last question on the survey, your participation will be complete.
• Your data will be combined with data from all the other respondents. The combined data will be entered into a statistical software program for analysis. The results then will be written up in a report, which may be published.

If you wish to receive a report of the results, you will have the option to request this by emailing the researcher, and a report will be sent to you electronically when the study is complete and all final results are available. Please see Dr. DiStefano’s contact below.

Are there any benefits to me if I am in this study?

There are no direct benefits to you from being in this study. However, by participating, you might help other people. The results of this study will provide information that could be used to better serve the social and health needs of diverse communities in Japan.

Are there any risks to me if I am in this study?

Some of the survey questions may make you uncomfortable, but you are free not to answer any questions that you do not wish to answer or stop participation at any time. In the unlikely event that you become significantly distressed, you can call the Ministry of Health, Labour and Welfare’s mental health consultation hotline at 0570-064-556 (toll free). If you are suicidal, you can contact the #InochiSOS hotline at 0120-061-338 (in Japanese) or the Tell Lifeline Japan at 03-5774-0992 (in English). Contact information and links to other services and organizations that can help with general medical and mental health problems, suicide prevention, and addiction also will be provided on the last screen of the online survey.

The survey is anonymous. So if you report illegal behavior (such as drug use; underage smoking, drinking, or gambling) or other sensitive information in the survey, the risk that it will result in any negative legal, economic, social, employment, or reputation damage to you or anyone else is extremely low.

Will my information be kept anonymous or confidential?

The survey data for this study will be kept anonymously. If you participate, Dr. DiStefano and his research team will not collect any information that could connect you to your survey data. This means nobody will know if you participated or not, nobody will be able to link you to your responses, and nobody will know who you are in the dataset.

If you answer all the questions in the survey and indicate that you wish to enter the electronic gift card drawing, you will be automatically sent to a separate webpage programmed and maintained by Dr. DiStefano. On this webpage, you will be asked to enter a valid email address. Your email address will not be connected to your responses in the survey, so all your survey responses will remain anonymous. You will not be asked to provide your name, so you may provide an email address that does or does not include your name. The webpage will not collect any personally identifiable information other than your email address (for example, it will not collect your Internet Protocol [IP] address). Your email address will be kept confidential to the full extent allowed by law. The study’s data will be stored on password-protected computers and on secure servers with strong firewall systems.

Are there any costs or payments for being in this study?

There will be no cost to you for participating in this study.

You are not guaranteed to receive money or any other compensation for participating in this study. However, if you answer all the questions in the survey, you will then be asked whether you wish to enter a random drawing for a chance to receive an electronic gift card for your participation. If you are randomly selected, you will receive an email from Dr. DiStefano’s study team with an electronic gift card. There will be at least two random drawings in 2025, each time selecting multiple participants to receive the gift card.

What are my rights as a research study volunteer?

Your participation in this research study is completely voluntary. You may choose not to be a part of this study. There will be no penalty for you if you choose not to take part. You may choose not to answer specific questions or to stop participating at any time.

Am I being asked to sign this information sheet or any other document?

No, you are not asked to print out this or any other document and sign by hand or provide your personal seal (hanko). You are also not asked to sign it with an electronic signature. When you click on the link to the survey, the first thing you will see is the same information you are now reading. To provide your consent to participate, you will simply need to tick a checkbox at the bottom of the screen and proceed to the second screen of the survey.
