Disciplinary Clearance Form

A Disciplinary Clearance, previously referred to as the Dean's Certification, confirms your student conduct record. This verification is required by various entities such as law schools, medical schools, travel abroad programs, state bar associations, government agencies, or independent agencies when applying for admission or employment. Typically, the institution or agency you are applying to or seeking employment from will provide you with a document that will need to be completed by a University Administrator. Please make sure to complete the form accurately.  Incomplete or inaccurate information will delay the processing of this document.

This does not serve as a certification of your attendance at Cal State Fullerton or a transcript. To obtain those, visit the  Registrar's office.

TIMELINE OVERVIEW

The processing time to complete the Disciplinary Clearance is approximately  10 business days  from receiving your request. Disciplinary Clearance Forms will be emailed directly to the institution or agency. Please ensure that you have included all of the necessary documents. Any missing or incomplete documents will delay the processing of your Disciplinary Clearance request.
