During your time at CSUF, you may request for various changes. This page provides a series of forms that you can access to make your requests. Browse by topic below. Each form includes information about why you should use the form, how to complete and submit it. Each form should be submitted to the corresponding office.

All forms submitted have different processing periods, not including the date received. Processing time may become longer during peak periods.

Admissions

 

These forms must be submitted to the Office of Admissions.

California Residency Forms
Form Description
CSU Nonresident Tuition Exemption Request Students who do not meet the requirements for California resident status but may qualify for the AB540/AB2000 nonresident tuition fee waiver should submit this form to the Admissions Office.
CSU Residence Questionnaire Students must complete this form to correct reported residency information or request a residence reclassification to California resident status.

 

 

Financial Aid

 

All completed forms and required documents must submitted to the Office of Financial Aid.

2025-2026 Financial Aid Forms
Form Description
Award Adjustment Request Form

Complete this form if you need any adjustments to your financial aid amounts. Reasons may include cancellation or reduction of aid, reinstatement of aid, and/or enrollment status changes.

If you have a Golden State Teach Grant (GSTG) award, you can also use this form to reduce or cancel your Federal Loans to make room in your budget for the full GSTG award.

Concurrent Enrollment Form Complete this form after census to report concurrent enrollment in CSUF and a community college during Fall 2025.
Cost of Attendance/Budget Adjustment Appeal Form Complete this form if you have additional expenses that are not being considered in the standard budget for the academic year Cost of Attendance. Make sure to review the eligible categories on the form.
Income Appeal Form Complete this form if you or your family's financial situation has significantly changed from the tax year used on your aid application, you may request a re-evaluation of your aid eligibility using more current income information.
Satisfactory Academic Progress (SAP) Appeal Form Complete this form if you have lost financial aid eligibility due to not meeting the Satisfactory Academic Progress requirements, and would like to submit an appeal if the cause was due to an extenuating circumstance.
Housing Plan Update Form Complete this form if you had a change in your housing. Submission of this form may result in modified a cost of attendance budget.
Teach Grant Certification Form Complete this form you are participating in the Teach Program and have completed the requirements to apply for a TEACH Grant.
Golden State Teacher Grant Certification Form Complete this form every year to get your Golden State Teacher Grant (GSTG) processed. 

 

 

Student Business Services

 

All completed forms must be completed and returned to the Student Business Services Office.

2025-2026 SBS Forms
Form Description
Stop Payment Application  This form is to request a stop payment on a check and to request the issuance of a replacement check.
Replacement IRS Form 1098-T Request  This form is to request a duplicate copy of Form 1098-T. This form is a “Tuition Statement” that educational institutions are required to provide to enrolled students who have reportable transactions.
Appeal for a Reversal of Fees Application  This appeal is to request that registration tuition and fees be reversed due to a drop or withdrawal from classes after a refund deadline.
