Online IT Request Form

All IT forms for telephone, email, wiring, and computer-related requests are now available online at: http://www.fullerton.edu/itrf

You must be an authorized Department Information Technology Coordinator (DITC) in order to submit a request. Instructions are available for each field of the form. Simply click the "Instructions" link next to the field you want help with. Technical problems with the online form can be referred to the Help Desk at extension 7777. Questions about filling out the form can be directed to Christina Yanez at extension 4488.

All instructions are in Microsoft Word format except the Button Templates.
