Repair Form
If your phone is dead or you are experiencing problems with your phone, use another campus phone and dial 4357 (HELP) and leave a message.
In your message, include the following information:
- Name
- Department
- Contact Extension
- Problem Extension
- Phone type (18 button, 30 button, or Voice Over Internet Protocol)
- Location of problem
- Do you have dial tone?
- Description of problem
Repairs received before 2 p.m. will be handled the same business day. Repairs received after 2 p.m. will be handled by the next business day.
