Eligibility
General principle: 14.01.3.
To be eligible to represent CSUF in intercollegiate athletics
competition, a student-athlete shall be in compliance with all applicable
provisions of the constitution and bylaws of the NCAA and all rules
and regulations of CSUF and the Big West Conference.
Some components of certification apply to all student-athletes,
while others apply separately to continuing student-athletes,
and incoming student-athletes (both freshmen and transfers).
Prior to the start of each sport season,
each student-athlete is required to attend an NCAA eligibility meeting
conducted by the Assistant Director of Athletics, Compliance.
At that meeting NCAA, Conference and institutional rules
are discussed and required eligibility forms are completed.
Administration of eligibility at CSUF,
which is under the direction of the Director of Athletics, includes
the Director of Academic Services, Eligibility/Admissions Coordinator,
Admissions and Records, Faculty Athletics Representative, Financial
Aid Office, Assistant Director of Athletics, Compliance, and other
related personnel.

All Student-Athletes
Student-Athlete
Statement-Form bylaw 14.1.3.1: Each student-athlete signs
this form at the initial team meeting prior to practice or competition
each academic year. Before
he/she signs this form the Summary of NCAA Regulations is reviewed
by the student-athlete in a meeting with the Director of Compliance.
These forms are placed in the student-athlete’s individual
file in the Director of Athletics office.
Drug-Testing
Consent-Form bylaws 14.1.4.1, 14.1.4, 31.2.3.1, and 30.5:
Each student-athlete signs this form and the Buckley Amendment
consent at the initial team meeting prior to practice or competition
each academic year. If
the student-athlete is a minor, this form must be signed by a parent
or guardian. Each student-athlete
will be given a list of banned drug classes and notified that changes
may be made during the academic year.
These forms are placed in the student-athlete’s individual
file in the Director of Athletics’ office.
CSUF
Academic 2.0 Requirement:
At the first team meeting of each academic year, the 2.0
requirement is explained. The student-athlete then signs the
2.0 document of Athletic
Department Academic Requirements stating that they have read
and understand the requirements and retains a copy.
Big West Sportsmanship Letter:
In an effort to promote integrity and a high standard of
sportsmanship among members of the conference, each student-athlete
is required to sign a statement acknowledging the value of fair
play, honesty, civility, and respect for opponents in competition.
His/her signature also attests that the student-athlete understands
the potential ramifications of non-compliance.
NCAA
International Student-Athlete Amateurism/Questionnaire & Statement:
To aid the institution in determining eligibility under NCAA
rules, each international student-athlete is asked, with the help
of an administrator if necessary, to complete each item on this
form. The eligibility-certification
official signs the completed form and keeps it on file with other
eligibility documents.

First-Time
Freshmen Student-Athletes:
This chart is sent to all incoming freshmen to assist them
with the eligibility and admissions process.
Admissions
and Eligibility Checklist:
This checklist is used by coaches to assist them in helping
student-athletes complete their admissions process.
Admissions
and Eligibility Checklist for Recruited Student-Athletes, First-Time
Freshmen: This
two-part checklist is used to assist freshmen with admission to
CSUF and eligibility certification with the NCAA Clearinghouse.
It is included with the packet provided by CSUF.
California
State University, Fullerton Notice of Admission: A letter from the Director of Admissions and Records is sent
to each student-athlete notifying him/her that he/she has been officially
admitted to the university.
Specific information regarding admission and test requirements
is provided in this letter.
Academic
Courses-Form 48H:
This form is used by the NCAA Clearinghouse for high school
administrators to list their school’s approved core courses, which
are used to calculate the NCAA core G.P.A.
NCAA
Initial-Eligibility Clearinghouse Certification Report (48C):
If student-athletes have been listed on the Institution Request
List and they have made application to the Clearinghouse, the institution
will receive certification status on these student-athletes as soon
as certification decisions have been made by the NCAA Clearinghouse.
Continuing
and Transfer Students
Permission
to Speak: Whenever
a student-athlete from another institution attempts to contact a
coach regarding a possible transfer, no discussion of any kind can
be held with that student-athlete until permission has been received
from the institution from which he/she wants to transfer.
To receive permission, the coach must make a request in writing
to the CSUF Director of Compliance, who will contact the institution
on his/her behalf. Only
when that institution advises the Director of Compliance in writing
that permission has been granted will the coach be allowed to speak
to the student-athlete.
Transfer
Residence Requirement Waiver:
This form is sent to the transferring institution from CSUF
after permission is granted to speak to the student-athlete.
The waiver statement advises that there is no objection to
the student-athlete being granted the one-time (4-4) transfer should
the student-athlete qualify and decide to attend CSUF.
JC
Transfer Student-Athlete Chart:
This chart, illustrating the admissions and eligibility process,
is sent to each junior college transfer student applying to CSUF
along with a letter of welcome from the Director of Athletics.
Admissions
and Eligibility Checklist for Recruited Community College Transfers
Needing an AA Degree:
This checklist is sent to incoming community college transfers
to remind them of the process at CSUF which must be completed for
athletics eligibility.
Request
for Exception Admission:
The Coordinator of Admissions & Eligibility for the athletics
department sends this request form to the applicable coach whenever
an incoming student-athlete does not meet CSUF admissions standards.
This form is completed and returned for review by the Athletic
Exception Committee.
Admission Status Report: The Coordinator of Admissions
& Eligibility prepares, updates and sends this report to the sport
coaches during the admission process for each term. A
copy is sent to the sport supervisor and academic services.
Walk-On
Eligibility Review: Coaches
submit student information on athletes who wish to become walk-on
members of their team. This
begins the process of eligibility certification for these potential
new team members.
Big
West Historical Report:
All first-time student-athletes at the institution must complete
a historical report to be reviewed and signed by the Institutional
Representative and Faculty Representative for certification for
competition.
NCAA
Squad List: Squad lists (head count/equivalency sports)
are compiled and updated by the Director of Compliance continually. These are sent to the Big West Conference office before competition
begins for each sport and updated at the end of the season and academic
year. In addition,
these are used for compiling graduation rates, NCAA revenue distribution,
and financial aid.
Eligibility
Status Report: An
eligibility status report, by squad, is completed, continually updated
and sent to the sport coaches by the Coordinator of Athletic Admissions
& Eligibility. A copy
is sent to the sport supervisor, academic services, training room,
FAR, Director of Compliance and equipment manager. The Student-Athlete's
eligibility is monitored from the time of admission to the
Faculty Athletic Representative's certification and continually
reviewed and updated during the academic year.
Participation
Report: At the
end of each academic year, a season of eligibility confirmation
form is sent to each head coach for the purpose of verifying final
squad list information.
Summer
Basketball League Permission Form:
In accordance with bylaw 30.15, in order for athletes to
participate in a summer basketball league, they must receive written
permission from the Director of Athletics to do so: bylaw 14.7.5.2a.
This permission form is for student-athlete use, to implement
this process.
Summer
Baseball League Permission Form:
In accordance with bylaw 30.15, in order for athletes to
participate in a summer baseball league, they must receive written
permission from the Director of Athletics to do so: bylaw 14.7.5.2a.
This permission form is for student-athlete use, to implement
this process.
Two-Year
Transfer Tracer: This
form is sent to the two-year transferring
institution from CSUF to aid in establishing remaining eligibility
of the transfer student-athlete.
Certification
Procedures for New & Continuing Student-Athletes:
In accordance with bylaw 14.10.1, this form is the CEO's
approved procedure for certifying the eligibility of an institutions
student-athlete under NCAA legislation.
Student-Athlete
Affirmation of Eligibility: This required form is used to
assist in determining eligibility. It is signed by the head coach,
director of athletics and faculty athletic representative.
When complete, this form is sent to the Big West Conference
office before the first competition.
Certification
Process: These
charts provide the coaches with graphic steps in the certification
process for first time freshmen, junior college transfers, 4-year
transfers, resident students and returning students.
