Academic Services and Sports Medicine Academic Services: In an effort to ensure academic development, the university provides counseling systems designed specifically for student-athletes. These services include academic advisement, guidance counseling, and daily study halls. Athletics academic services provides a handbook for student-athletes describing in detail specific areas of its program. Other aspects of the academic program include guidance in the following areas: A. Priority Registration B. Trailing NCAA Eligibility C. Processing of Fee Waivers D. Overseeing a Book Voucher System F. Strategy Classes G. Faculty and Peer Mentoring H. Academic Recognition I. Tracking Normal Progress with FAR J. Academic Disqualification Process and Reinstatement CSUF Policy for New, Continuing, and Disqualified Student-Athletes Receiving Services: A review of bylaw 13.2.7 is provided for coaches and staff to clarify allowable services to student-athletes in this category. Sports Medicine Program: The athletics department of CSUF is concerned with the health care of all its athletes. The responsibilities of its athletics training program include prevention, evaluation, referral, treatment, and rehabilitation of athletics-related injuries of student-athletes who are enrolled and on an official team roster, and certified to practice and/or compete. Lists of teams are provided by the Director of Compliance on a regular basis to keep sports medicine staff up-to-date on the status of all student-athletes. To ensure that student-athletes receive complete medical benefits from health-care personnel and that appropriate information is on file in the training room, all student-athletes are required to complete the following medical information forms: medical insurance form, athletics health-assessment form, and the emergency information card. This information is collected before student-athletes start practice in the fall. Policies and procedures of the training room are on file with the Head Athletic Trainer. The sports medicine program works closely with the CSUF health center and area physicians to provide the best possible care for student-athletes. A. A new student may receive no services before being admitted
to the university. B. Continuing student-athletes may receive services and equipment up to the last day for enrollment (approximately 3 weeks into a semester)-after that date, if they are not enrolled, they may not receive services. C. A disqualified student-athlete may be considered a continuing
student while he/she is petitioning to enroll (up to the last day
he/she is permitted to enroll). Flowchart for Certifying Continuing Eligibility: Using the principles set forth in this flowchart provided by the NCAA, the Faculty Athletics Representative (FAR) calculates the eligibility of each returning student-athlete. Initial Eligibility Waiver: This application, completed for the NCAA council subcommittee on initial-eligibility, along with the NCAA initial-eligibility clearinghouse final certification report, must be submitted before the NCAA can process the waiver request. Satisfactory Progress Waiver: This application must be completed and submitted with a letter of explanation to the NCAA subcommittee before the waiver request can be processed. Medical Insurance Information: This is a required information package to be filled out by each student-athlete before practice or competition and kept on file in the sports medicine office. The package consists of a medical insurance information form, acceptance of risk statement, statement of understanding signature sheet, and assessment form. Emergency Information Card: This is a required information card which is completed by each student-athlete and carried to all away events by the athletics-training staff. Athletics Training Procedures: The summary of medical insurance policies and health-screening and physical-exam procedures for student-athletes. This sheet is explained and provided for the student-athletes at their yearly initial meeting. NCAA and Big West Medical Hardship Waiver Bylaws: a summary of bylaws which apply to medical hardships is given to the coaches each year. Big West Conference Hardship Waiver Petition: This petition must be completed and submitted to the Big West Conference commissioner before a hardship waiver can be considered by the conference subcommittee. Academic 2.0 Appeal Petition: This petition must be completed and submitted to the Assistant Director of Athletics, Academic Services. It will be reviewed by the academic standards subcommittee of the athletics council which will render the disposition of the appeal. Academic Appeal Petition Committee Procedures: Procedures include membership, purpose and procedures. Policy
on Physical Exams: Prior to practice, conditioning,
or competition, all student-athletes must complete a physical examination
following the guidelines set by this policy. ![]() |
