Sports Medicine Program: The athletics department of CSUF is concerned with the health care of all its athletes. The responsibilities of its athletics training program include prevention, evaluation, referral, treatment, and rehabilitation of athletics-related injuries of student-athletes who are enrolled and on an official team roster, and certified to practice and/or compete. Lists of teams are provided by the Director of Compliance on a regular basis to keep sports medicine staff up-to-date on the status of all student-athletes.

To ensure that student-athletes receive complete medical benefits from health-care personnel and that appropriate information is on file in the training room, all student-athletes are required to complete the following medical information forms: medical insurance form, athletics health-assessment form, and the emergency information card. This information is collected before student-athletes start practice in the fall.

Policies and procedures of the training room are on file with the Head Athletic Trainer. The sports medicine program works closely with the CSUF health center and area physicians to provide the best possible care for student-athletes.
